Diagnosis of Symptoms: Constructive Criticism on My Poem? :)?

It’s called “Symptoms”. It’s about my diagnosis of NVLD, a mild form of Asperger’s.
Lines on a page, black on white,
form words that mean nothing to most,
but everything to me.
It’s like a recipe-book of ingredients that are fine on their own
but not together,
or a map to a place that’s so hard to get to
that the map barely even helps.
Just words, just words.
To anyone else, just words.
But to me, these phrases are old friends
that I’ve always been vaguely aware were around
but never took the time to acknowledge.
Each one holds a memory,
a story that was long forgotten but makes sense now.
My life, my peculiarities, are spelled out in front of me
like this person watched me grow up and wrote down what made me tick.
It isn’t easy.
It isn’t easy to see these lines on a page,
and realize that what I thought was my personality
is something doctors are striving to fix.
